Incident Reports

We know that in order to grow, we must listen – so we invite our community to hold us accountable in this process.  If you have experienced an incident related to equity, accessibility and/or inclusion that had a negative impact on you, please use the form below to report it to us. Incident Reports can be submitted anonymously and will be reviewed promptly by our employee IDEA team. If you would like someone on our team to follow up with you, please provide contact information.
